- If your child has Mass Health insurance, and you need transportation to a medical, dental, or therapy appointment, please let us know. We can complete a prescription for transportation (PT-1) for you. You will receive a notice from Mass Health upon approval, with instructions for how to schedule transportation. Please note that rides must be scheduled 48 hours in advance of the appointment.
